Transitions of Care Workshop

CSHP-UCSD hosted its annual Transitions of Care workshop on February 24, 2016. Dr. Michelle Schlueter and her PGY2 Transitions of Care residents, Dr. Faaiza Alibhai and Amrit Dosanjh, as well as Dr. Winnie Thi from Scripps Mercy attended the workshop. Dinner was catered from Lorna's, and the pharmacists gave a short presentation about what Transitions of Care is, and explained to students how the program works at UCSD and Scripps. The students then broke up into small groups and worked up a patient, and had time to ask the pharmacists and questions they had.
